b) address, phone, fax
c) auto validate form that prompt thankyou and error page, also sends inquiry email to both visitor and administrator.
Here are the site creation SOP steps:
1. pick and download a CSS template off openwebdesign
2. set up the project site on Dreamweaver CS3
3. create template .dwt file
